What to Expect: Your Experience


 

Initial consultation

Initial appointments are 2 hrs with your physician. Prior to your first appointment, you will be asked to fill out comprehensive health history questionnaires and provide pertinent medical records. Your intake questionnaires and medical records will be reviewed in full by Dr. Susan Bradt prior to your first appointment.

During your initial consultation, Dr. Bradt will review your history in person, expanding the scope to gain a better understanding of your health timeline and your current concerns. Hearing your health story is integral. She will integrate your story, health timeline and illness history to help bring insight to the triggers and mediators of your health concerns.

Conventional laboratory testing is typically combined with functional medicine testing arranged at your first visit.  Our office staff will guide you through the process of testing and follow-up. Dr. Susan Bradt will provide an initial Care Plan while awaiting results of the laboratory testing. Initial Care Plans often include dietary and lifestyle modifications, basic nutritional supplements and specific treatment for the most pressing concerns.  


First follow-up

Appointment is typically 90 minutes and is scheduled 6 weeks after your initial intake. All initial test results are reviewed in full and put in the context of your health concerns. Your progress is evaluated and your Care Plan will be personalized and modified based on test results and progress.


Ongoing
follow-up

Ongoing follow-up appointments are typically 30-60 minutes when we will assess your progress, review follow-up testing and modify your Care Plan.  

 


Care Plans

A comprehensive Care Plan Summary with all recommendations along with specific handouts are provided through your patient portal with every visit.

Your Care Plan will include recommendations such as:

  • Specific lifestyle modification (addressing sleep and relaxation, stress, movement, relationships and nutrition)

  • Nutrition recommendations and specific food plans tailored to your needs (for autoimmunity, glucose regulation, cardiac health, adrenal support, etc.)

  • Tailored nutraceutical or herbal recommendations

  • Referrals to other providers such as counselors, acupuncturists, physical therapists, health coach/nutritionist, etc.


Communication

All established patients have an easy and convenient patient portal.  Here you can communicate securely and directly with Dr. Bradt or her staff, review your lab results, upload documents for your physician to review, update your current supplements and medications, enter home vitals, review handouts and much more.
